11. The applicant must, once the layout of the premises is complete, provide a certificate attesting to its compliance with the plans approved by the Minister in accordance with section 19 of the Act. The certificate must be issued by an architect or any other professional accredited by law to issue it.

11. An applicant must, if applicable, include the following information and documents with the application:
(1)  a certified true copy of its incorporating act;
(2)  a copy of the declaration of registration or initial declaration entered in the enterprise register under the Act respecting the legal publicity of enterprises (chapter P-44.1) and any declaration amending the declaration;
(3)  a certified true copy of the resolution authorizing the application;
(4)  the name and address of the residence of each member of the board of directors and each shareholder and, where applicable, their position as an officer of the legal person; and
(5)  the name and the address of the residence of each related person who is a permit holder.
